ACHC offers a Sliding Fee Discount program! All patients are eligible to apply, even those who have insurance. Enrollment forms are available on the link here or in the clinic. Applications must be renewed annually and anytime there is a change in an enrollee's financial status. It is preferred to have completed applications prior to your appointment date. Paperwork takes approximately 30 minutes to complete and you will need your SSN, Insurance cards, Driver License or Identification to complete the documents. ACHC has Certified Enrollment Counselors on staff to assist you with enrolling in the Marketplace. If you have recently moved into or within the State of Idaho; lost coverage due to a job change; had a child or gotten married / divorced, you may be eligible to enroll in a qualified health insurance plan!
