ACHC offers a Sliding Fee Discount program! All patients are eligible to apply, even those who have insurance. Enrollment forms are available on the link here or in the clinic. Applications must be renewed every six months and anytime there is a change in an enrollee's financial status. It is preferred to have completed applications prior to your appointment date. Paperwork takes approximately 30 minutes to complete and you will need your SSN, Insurance cards, Driver License or Identification to complete the documents.
