History & Mission

WE LOVE WHAT WE DO

Your health care team will work together to improve your health- across all modalities of care.

Adams County Health Center Inc. has reached  NCQA Level 2  recognition.  We are dedicated to increasing the quality of care and focusing on what the patients needs and educating the patient on how to achieve their health care goals.

Starting out as the Council Hospital, meeting the full service healthcare needs for our rural community.  Due to lack of funding, the hospital morphed into a skilled nursing home for area residents to reside with round-the-clock care.

In March, 2002, the hospital and nursing home closed, leaving the small, financially unstable clinic.  By April, 2002, the clinic became designated as a free-standing Rural Health Clinic.  In December 2004, ACHC became a Federally Qualified Health Center and eligible to receive additional funds and enhanced reimbursement opportunities.  Home to approximately 3700 residents and encompassing 1,365 square miles, Adams County is designated as a Primary Care, Dental and Mental Health Professional Shortage Area, as well as a Medically Underserved Population area.
ACHC has grown substantially and is now the largest private employer in Adams County.

Additionally, ACHC is continuously expanding our services to meet your needs. We have routine and urgent medical care services, dental services, optometry services, laboratory and digital x-ray services and now a full service telepharmacy - all open Monday - Friday.
